Abstract: Background The importance of primary cardiac rehabilitation after PCI has been confirmed, but the adherence of patients is poor at present. Therefore, exploring the adherence and influencing factors of PCI patients with phase I cardiac rehabilitation can provide a theoretical basis for improving patient adherence. Objective Through the construction of structural equation model, the influencing factors of PCI patients' phase I cardiac rehabilitation adherence and the action path between these factors are clarified, which provides theoretical support for improving PCI patients' phase I cardiac rehabilitation adherence. Methods Using the method of convenient sampling, 430 patients with phase I cardiac rehabilitation after PCI in a grade A class III hospital in Harbin were investigated by General Demographic Data Questionnaire, Cardiac Rehabilitation Adherence Questionnaire, Coronary Heart Disease Health Belief Questionnaire, Anxiety and Depression Scale, Family Intimacy And Adaptability Questionnaire, Chronic Disease Resource Questionnaire, and structural equation model was used to analyze the relationship between influencing factors. Results The structural equation model constructed in this study has a good fit, and health beliefs, family intimacy and adaptability, and the utilization of chronic disease resources have a direct positive impact on the adherence of phase I cardiac rehabilitation( β= 0.395、 β= 0.277、 β= 0.152,p<0.01); Depression and anxiety have direct negative effects on adherence( β=- 0.055、 β=- 0.166,p<0.05); Health beliefs, family intimacy and adaptability can have an indirect positive effect on cardiac rehabilitation compliance through the utilization of chronic disease resources( β= 0.057、 β= 0.065, p<0.01), and it can also have an indirect negative effect on cardiac rehabilitation compliance through anxiety and depression. Conclusion The adherence of PCI patients with phase I cardiac rehabilitation is affected by many factors, and there are complex path relationships between these factors. Health beliefs, anxiety, depression, family intimacy and adaptability, and the utilization of chronic disease resources all play an important direct or indirect effect in the adherence of PCI patients with phase I cardiac rehabilitation.
